Using personalised care to connect you with services that can support your health and wellbeing:

• To support your mental wellbeing
• To be more active
• To support you with your finances
• To live well and more independently
• To be healthier
• To be more socially connected

Mental Health Occupational Therapist

Support patients with a wide range of mental health conditions to enjoy everyday activities that lead to a sense of purpose and enhance their chances of recovery. 

This could be to:

• improve self-care, develop more purposeful and effective routines
• learning to cook healthy meals
• managing finances better by developing a budget
• building confidence to go out again, talk with neighbours or join a local group. 
• helping people to develop professional skills

HOW TO ACCESS 

A referral can be made to any of these services by contacting your GP Surgery and any member of staff can refer you.
